[whatwg] "live" image maps

I think the specification should be clearer about what happens when an  
<area> element is added or removed. When an <img> element is added with a  
usemap="". When the shape="" attribute is altered, et cetera. Either by  
handling each case specifically or stating in general that the specified  
algorithms always apply or something.
