Implementation should be left up to the browser author, but it would probably
be useful if the spec lists some suggestions. Both to help browser authors
understand what the spec means, and to help content authors realise that how
their favourite browser behaves may not be how some other browser behaves.

I agree it would probably be important to state that the UI must not take up
extra space. (An author-provided UI however should be allowed to take up
author-defined space.)

Btw, please let's not have the spec say "right-click", when "contextual menu"
is meant. How to bring up a contextual menu is entirely
environment-dependant: my mouse has no right button at all, but I use
contextual menus all the time. Also, my preferred OS offers a native "Action"
button/menu (typically in a toolbar) to contextually provide access to
actions. Another possible UI would be straight from the keyboard, as is
already the case with the QT plug-in for example. The mouse hover-based UI of
QT and VLC that has been mentioned is probably another good example (although
requiring users to hover over the video will mean many won't ever discover
this functonality, just like the contextual menu -- I imagine this is what
Apple's Action button aims to solve).
