Why is it okay for a document to not specify its HTML version?  
Do all browsers ignore it?  

How should a validator handle lack of HTML version info when the next
standard is released with no DOCTYPE?

Why aren't MIME type version numbers included in HTTP Accept headers?
Looks like it would be a valid HTTP/1.1 "accept-extension": 

  Accept: text/html; q=0.9; v=4.01, application/xhtml+xml; q=1; v=1.1, */*; q=0.1

I don't see anything about registering accept-extensions.
