I know of such an example. It was an intranet app for consultants to
record their billable hours. It did browser sniffing to determine
whether IE was available to allow a COM-based date control to be used
in place of a plain text input control. I didn't write it.

The form did not use the label element. In the IE case the COM object
looked so much like a date input widget that the author didn't even
label it with plain old text.
